All athletes 7-12th grades in Iowa have to have a current physical (within the last calendar year) on file to participate in athletics.

All athletes must review and sign off on receiving the “Heads-up Concussion Form”

All athletes must review and sign off on the Student Driving Permission form for inter-city practices and competitions not requiring school bus transportation

All athletes that are failing a class at a grade check time (not end of semesters) must have their teachers fill out this eligibility form to regain eligibility.
