Required Immunizations

 

Parents & Guardians of Children entering Kindergarten: Does Your Child Have the Shots Needed to Start School?

Elementary School Requirements 4 years of age and older:

5 doses Diphtheria/Tetanus/Pertussis with 1 dose received > 4 years of age if born on or after Sept. 15, 2003; or 4 doses, with 1 dose received > 4 years of age if born after Sept. 15, 2000, but before Sept. 15, 2003; or 3 doses, with 1 dose received > 4 years of age if born on or before Sept. 15, 2000.

4 doses of Polio with 1 dose received > 4 years of age if born after Sept. 15, 2003; or 3 doses, with 1 dose received >4 years of age if born on or before Sept. 15, 2003.

2 doses Measles/Rubella: the first dose shall have been received > 12 months of age; the second dose shall have been received >28 days after the first.

3 doses Hepatitis B if born on or before July 1, 1994.

2 doses Varicella > 12 months of age if born on or after Sept. 15, 2003; or 1 dose received >12 months of age if born on or after Sept. 15, 1997, but born before Sept. 15, 2003, unless the applicant has a reliable history of natural disease.

7th Grade
Iowa Immunization law requires that all students entering 7th grade are to have both a Meningococcal vaccine and Tdap (Tetanus, Diphtheria, and Pertussis/whooping cough) booster shot before the first day of school unless the student has a Certificate of Immunization Exemption. Any student who is still not compliant by the first day of school will be excluded from school in accordance with Iowa Code 139A.8. Please know, it is the parent/guardian's responsibility to provide proof of vaccination or the Certificate of Immunization Exemption to the school.
 
12th grade

The Iowa Department of Public Health requires that all students entering 12th grade must receive the Meningococcal vaccine before the first day of school unless the student has a Certificate of Immunization Exemption.  Please know, it is the parent/guardian's responsibility to provide proof of the vaccination or the Certificate of Immunization Exemption to the school.
