Dental Screening

Family Inc. and the I-Smile Program is excited to tell you about a FREE dental sealant program in your child’s school.This program offers your student a dental screening, a fluoride varnish treatment, dental education and dental sealants at school with no cost to you.
The Facts:
-A dental sealant is a plastic material this is placed on the chewing surfaces of back teeth. This “coating” helps protect teeth from germs and food that can cause cavities. They are very effective at preventing cavities and are recommended by the American Dental Association as well as the U.S Surgeon General.
-Fluoride varnish makes teeth stronger. It can help prevent new cavities as well as stop cavities that have already started.
-A dental screening is a visual screening. No instruments are used and no x-rays are taken.
-All services will be provided by a registered dental hygienist employed by Family Inc.
-Results of the screening will be sent home to parents on the same day.
-These services do not take the place of regular dental checkups at a dental office.
What We Need From You:
Please fill out the consent form below and return to school with your child’s registration forms at “Back to School” night or the first day of school.
